[Bolus, Lantus] Diabetes Technology Blog: Symlin and what I know about using it
Ill usually inject insulin using an extended (square wave) bolus about an hour after eating to take 20% less insulin over a 90 minute period. I never take Symlin if I have more than about a unit of insulin on board from a previous bolus. If Im about to eat and my blood sugar is high, then Ill partially correct for that with a regular bolus and then use my extended bolus after eating. when using Symlin just as a mealtime bolus through subcutaneous injection.

    [Bolus, Lantus] Bad day
I have my Lantus , but not my short-acting insulin, the one I take with meals. Then I remember taking it out of the bag the night before so I could take a correction bolus before bed. So a couple weeks back I did something really stupid. I went to work. and left my insulin at home. Yeah, I told you it was stupid. Problem is I don't work in a normal office job where I could just take my lunch break and run home and get it. Nope.

I reduced my bolus to half and took my regular Lantus as scheduled. The days leading up to this race were pretty horrible. Ryan was down with the exorcist flu. Explosive purging from both ends constantly. I left work early to take him to the ER. The next day I took off work to take care of him. He was wasted. Little did we know it would be 5 more days before he was able to hold anything down. You can read his story HERE if you can handle it, that is.

With lantus, Addison would have one injection of long acting insulin that may or may not last 24 hrs in his body. It has been accurate as far as reducing or adding insulin for a bolus only a couple of times. Here is another question for all you pump experts - when you have a low, do you enter the low number into your pump to calculate a bolus or do you wait and enter the number after it has gone up?

Far too much advertising for insulin, (especially ads for Sanofi Aventis Lantus product) might even be viewed by some as offensive to type 1 patients -- talking about insulin as a wise "choice" in treatment. Type 2 diabetes (simplistically) is abnormal production of basal insulin while bolus capacity has been eliminated. No matter the method of delivery, fast-acting bolus insulins will have an unpredictable peak based on each individual's biochemistry.
